Vanderloo Family Medicine is excited to announce that we are now offering Direct Primary Care (DPC) Memberships!
Direct Primary Care (DPC) Memberships offer a substantial savings, in addition to a greater degree of access to, and time with, your physician. Memberships include appointments for wellness exams, acute and/or chronic disease management, and access to telehealth when the physician deems clinically appropriate.
Vanderloo Family Medicine offers several membership types to fit the needs of every patient:
Adult Individual Membership, for ages 19+: $65.
Child Membership, for ages 18 and under: $35 (must have at least one adult member enrolled).
Family Membership, including 2 adults and 2 children: $165. Additional children may be added: $20 each child, up to a family maximum of $205.
Student Membership, for full-time students only, proof of enrollment required: $45.
